Christa Pike remains alive after Tennessee officials failed to carry out her execution with two doses of lethal injection, turning a scheduled death sentence into a case that could reshape the state’s remaining execution plans for 2026.

Pike, 50, was scheduled to be executed at Riverbend Maximum Security Institution on September 30 for the 1995 murder of 19-year-old Colleen Slemmer. Tennessee officials administered two doses of pentobarbital, but Pike did not die and was later transported to an outside medical facility.

The decision means Tennessee will not proceed with the other execution that had remained on its 2026 schedule while the review takes place.

Pike’s case is the second failed Tennessee execution this year
